I bet testers didn't find this right away because of the scope of a free modded project like this, people aren't going to do completionist test runs, especially in a Bethesda engine. If people are doing parts of the character in a set of tasks that include lots of other characters, I can see how dialog or model making didn't add up to what the combination would be.
